If the cancellation is a result of sickness or injury, please include the medical certificate fully completed by the treating physician. The physician who fills out the medical certificate must be the same one who treated the sick or injured person on the date of the incident that caused the cancellation. If the interruption is the result of a sickness or injury, provide a letter or other document showing personal attendance by a doctor during the trip, the date and nature of the illness, details of the treatment and the doctor’s declaration that your trip was interrupted for medical reasons, for either yourself or other covered individuals.

If the cancellation or interruption is the result of a death, please attach a copy of the death certificate of the deceased person.

Attach a copy of all sales invoices and receipts as proof of payment for your covered trip showing date(s), amount(s) paid, travel supplier fees and penalties, and the method of payment for your insurance. This is required for all the deposits and final payments you made to your travel supplier.

Attach a copy of all unused travel documents (e.g., tickets/e-tickets, cruise booklets, travel vouchers, etc.), as well as the terms and conditions of booking, if applicable.

Attach a copy of the cancellation invoice issued by the travel supplier(s) that confirms the penalties and/or cancellation fees applicable to the cancellation of your trip (if applicable).

Provide any proof that you requested a refund from your travel agent, tour operator, common carrier, hotel or any other organization to whom you made a prepayment prior to your trip, for unused tickets, vouchers, confirmation notices etc., and that such a refund was denied; or provide proof of any travel refunds received (e.g., credit card statement, explanation of benefits showing amounts received, etc.).
